GDPR Risk Assessment Form

There's been a lot written lately about the new General Data Protection Regulations (GDPR) which became law in the EU yesterday, Friday 25th May.
I'm sure, like me, your e-mail inbox has been inundated by Privacy Policy updates from everyone you have subscribed to online, and while this may start to get a little tedious it can be a great framework for wording our own Privacy Policies on our own niche websites.
I've been printing them all off and am now going through them picking out the wording that I like.
One of the requirements of GDPR is to have a Risk Assessment with which you can show anyone who requests the details of their personal data that you hold, how you store it and how you use it.
Data has to be securely kept, whether it be electronically on a password protected device or physically in a fire proof container with a combination lock.
I've created a Risk Assessment form which I think covers pretty much any eventuality for basic personal data. It's generic and there would be additional things to add to it if, say, you are promoting Health and Wellness products and services. In this case, you may need to include a bit about your Client's medical history.
If or when someone asks you for details of what data you hold on them or reports you for a possible data breach, you will need to provide them and your relevant national authority with that information.
If you're collecting personal data via a contact form on your Auto Responder, take a snapshot of the contact form showing your GDPR compliance statement and their consent and keep it with your Risk Assessment form you have created for that person.
