Amazon can be very awkward to work with if you don't read all the fine print and I know a few people who have lost money by being banned for not having the official Amazon affiliate disclaimer on their sites.
I have gone through all the what's, how's and where's of the Amazon affiliate programme in the EU and also been speaking to them via e-mail.
I have also been speaking to amazon.com for the U.S one aswell
If you are promoting any Amazon products in the EU or the U.S on your websites it is essential that you have their own disclaimer as well as your own on your site, just to cover your ass.

I have never put affiliate disclaimer on my site till now, but to be on the safer side and since my AdWords account just got suspended recently, I am thinking of creating a menu (probably footer) as "policies" and put Amazon's, Leo's one and privacy policy all as sub-menus to it.
