This is the official EU Amazon disclaimer that they sent to me via e-mail which you can copy and paste it into your websites.
If you are not within the EU you won't be able to use this but that's all you need to do is contact your AMAZON AFFILIATE PROGRAM and ask for the disclaimer to cover where you live.
Make sure you put your websites name into the start and end of the disclaimer instead of [insert site name]

Thanks for the suggestion but the link you have provided is not working for me.
I have never put affiliate disclaimer on my site till now, but to be on the safer side and since my AdWords account just got suspended recently, I am thinking of creating a menu (probably footer) as "policies" and put Amazon's, Leo's one and privacy policy all as sub-menus to it.
