I have copied this with my site name into a new page named "Amazon affiliates disclaimer" and published it with no tags or description.
You can then put it anywhere on your website but I have put it into a custom menu with my other legal stuff at the footer of my main website page.
if you need help creating a custom menu there is plenty of great training here for that, just type in the search bar "create a custom menu" and you will find some.

Thanks for the suggestion but the link you have provided is not working for me.
I have never put affiliate disclaimer on my site till now, but to be on the safer side and since my AdWords account just got suspended recently, I am thinking of creating a menu (probably footer) as "policies" and put Amazon's, Leo's one and privacy policy all as sub-menus to it.
