I was thinking to use the how to lose weight and back pain but it has only 36 searches and even though there is 0 competition I have already done many articles with low searches and competition and have very little traffic. That's why I try to find keywords with more searches.

A keyword with zero competition should generate visitors to your site. Like everything else its a waiting game.
You could try to hurry it along by posting on Social Media Groups and on AD sites.
You could even try a small PPC Campaign.

Maria, I've been there and got rid of some websites. You have to follow your heart. You'll know what's best for you. I agree with Loes that backpain is negative and you need a website that you dream about and when you wake up are eager to add content. Just my 2 cents. I wish you much success whatever direction you decide to go.

I haven't really thought of it as a negative subject because I write very variable content (maybe too out of topic...) for example how food choices affect back pain and which supplements are good for back pain etc...

Have you tried to think about this in reverse? Instead of finding keywords and then figuring out what to say about them, try focusing first on what's in your heart and on your mind. You can then write freely and find keywords that go with your topic afterwards, adding them wherever they fit naturally.
Think about things that you naturally have in your head, are experiencing, or have seen others experience around you. Forget about what other people have already written. You want to be unique. You have something that no one else can offer because no one else is you. Once you have something that you want to say, ask a question: how can you tie that back to your niche?
Adding some personal perspective will bring some life to your posts, and it will make writing easier because you're not just regurgitating facts or repeating ideas that have probably been written a million times. You're putting a piece of you into it, and you'll connect with your readers more if it feels natural to you.
If nothing you naturally think about and experience has anything to do with back pain, then you should ask yourself why you chose that niche. If you just chose it because there's a large audience for it and might be profitable, that's not the right kind of motivation to inspire your writing. I think you'll find it easier and eventually more profitable to find a niche that you can write about in your sleep because you're already thinking about related topics or they relate to your life in some way.
Find the right niche, and you will be able to come up with a long list of potential topics without even looking at keywords. That comes from knowing your topic well and feeling passionate about the message you're putting out there.
There's nothing wrong with starting over! You can keep the first site around and add to it whenever you feel inspired, but you should also give yourself the gift of starting something new that will spark your passion more.

I first started back pain related site because I have suffered from back pain and I managed to get rid of it. I also knew which affiliate program to use because I had already used some of their products and they were effective. So back pain seemed like a good option.
My only problem when writing content until now has been the fact that I am very slow. And partly it is because I am not really and expert of anything (besides horses but I am not passionate about them anymore.) So now that I tried to write two posts a week it got really hard.
I totally understand what you say about writing from my own experience and I actually could write my latest post in record time doing so but it was only 400 words. And I think that if someone looks information about eliminating back pain they will expect to find some proven or scientific information not just my personal experience... I think the best way would be integrating those two.
I am not a person who talks a lot and maybe I could not speak all day long about any subject. Perhaps I should do few smaller niche websites (like Jay's football snack helmets) instead of one big authority site... I'll try to figure out what would be the best solution for me

(Of the record: You need to find a subject, you can talk all day about. And spiritually seen, it's not a good case to talk al day about backpain, very negative)

Children's toys, education, parenting, clothes, shoes, swimming lessons, picture books, potty training, hair cuts, measles, growing pains, healthy food, traveling, etc.etc.

Many have heaps more experience and advice than me so this is mostly and encouragement. I have a similar problem. Not being an expert in my chosen niche I struggle to write content.
I find the best way is start writing but it is better if you have some idea or theme.
I find that having started and write until I need to stop I can then edit what I have written most times I find that while my content is rough and a draft I have something to work with.
You need to decide if you want to focus on something else as only you know the value of this.
My bit of encouragement is just write let it flow and see what results.
