I use "contact form add" which I have placed in my right sidebar.
When visitors sign-up the plugin allows me to send an automatic email. In the email you can give the link to download the e-book.

On your WP dashboard go to the plugins and install:
Email Subscribers & Newsletters. This Add subscription forms on website, send HTML newsletters & automatically notify subscribers about new blog posts once it is published.

Activate it, go to the widgets and add widget.
