These are the questions I ask myself every day. I usually answer the questions in the evening before I go to bed. Notice there are no yes or no questions. They are all open ended. Once found what worked for me I don't change them.
Notice the questions have two purposes. Some of them help me define what is important to me and what I need to work on today. Some are psychological. The help me identify negative thoughts that may be interfering with my accomplishments. they ask me how I feel about my day and what I accomplished. They ask me to replace any negative thoughts with positive ones that will help me move forward.
Try them. Adapt them to you life and let me know what you think.

Which specific daily action(s) that relate to your goals did you work on?
How Many Minutes did you spend on your A level task.
What do you feel good about
What challenges did you have?
Did you notice an negative self-statements
Any postive statements to replace the negative
What are your specific plans for tomorrow that will move you toward your 4 week goal.
How do you feel about your day on a scale of 1-10 . (1 is a junk day-- 10 it couldn't have gone better.)
What did you learn today.
I didn't initially catch the importance of this but realize the brilliance of keeping it simple. I know of others that are a whole lot better...

Yet another method I like that I started implementing effectively just over the last week is the one goal a day method: http://www.onesimplegoal.com/tips
Using this method, I finally got myself to do one thing I've been putting off for ages, i.e., meditate (using YouTube guided meditation video). And yesterday, I finally got myself to figure out how to use a drill.
One thing that doesn't work for me is trying to stick to a schedule. Though a schedule might work for some people, I seem to be resistant to any fairly rigid structure.
Setting daily goals are so very important. That way a dream can become a reality.
