First of all, congrats on jumping into all of this. Sounds like you're a talented guy! I think you've gotta think about a diversity of offerings if what you're wanting is a truly passive income.
What I mean is, you need to do a little PPC, some low cost affiliate marketing with SEO, some high dollar, big ticket marketing as you build a mailing list, and even consider rolling out your own products.
On the fitness side, you could do some Youtube demos of various exercises, and in the description, link to a product that you trust from the Clickbank or JVZoo marketplace.
On the app dev side, you could become an affiliate for sites like Lynda, Teachable, and Udemy and link to their courses as an affiliate.
Those are just little thoughts. I can't speak to the best model when it comes to actual app development, but I'd consider those avenues, at least on the side.

Personally, from what you describe, it sounds like a very uphill climb you're working against where you need a lot of time that you don't have. Are you sure this is the direction you want to go in? There are easier niches to get into. It's up to you of course, but your own description sounds like this is an extremely challenging area to make money with.
