And you add a contact form. This is done with a plugin.

Create a contact form

There are multiple plugins for contact/subscribe forms

  1. Go to plugins
  2. Add new
  3. Search contact form
  4. Choose a compatible one

5. Test your form!

6. Add the short code for the form under the text on your page

quick question---our site mngr says we shouldn't have more than 5 plug-ins-which effects our site health--can we get around that or should we just ignore it?

I believe, when you've finished the certification course you are above 5. Always be careful with adding plugins, but 5 is a bit low.
I can tell you, my site health for plugins gives on all my websites 0%
