You place your keywords in your site content - as headlines, etc. You should reference them in the first paragraph of posts, articles, etc., but write naturally. Home title and description are just that, so unless your Site Title is your main keyword, you shouldn't have to worry. Don't try to cram everything into that, Google will penalize you. The description should be a brief description of your site and the information it provides in conversational English, not keywords.
