You have two options ...

1. Adding the login/registration modal to your main menu structure

Go to Appearance = > Menus, and simply add to login/logout/register pages to your primary menu:

And here's the live result in my case (after clicking the "Login" link):

Of course, if you want you can create a separate dedicated login page using the shortcode displayed on the plugin configuration page:

2. Adding the login/registration modal to your sidebar

Go to Appearance => Widgets and simply drag the CSH login widget into the sidebar widget area:

And here's the result:

Cool isn't it?
