We are going to use a handy plugin called Ask Me Anything.

Note: you'll see the usual warning message saying that the plugin hasn’t been tested with the latest major releases of Wordpress ... Ignore it ... I've tested the plugin on Wordpress 5.1 and works fine.
So ... Basically, the plugin will allow you to create a page with a short-code based form where users can ask you questions anonymously. It lists the questions and answers neatly, and there are 20 useful, easy-to-use attributes that will allow you to easily customize your shortcode according to your needs.
The plugin will save the questions as Wordpress comments. So comments have to be enabled on the given page! And of course, you can use the shortcode on a text widget by using the page ID in the post_or_page_id attribute ...
Depending on your settings (Settings => Discussion), new questions will be held for moderation and you will be notified via email.
It's a small plugin (128 KB), but let's see the usual speed tests ...
