You need to complete parts 1 and 3, and then sign it.
Did you complete all the boxes, including name, address, date of birth, and foreign tax identifying number?
It's possible that the affiliate program has geographic restrictions and does not allow your country.

while W9 form is used by US citizens to provide their taxpayer (TIN) to entities that will pay them income during the tax year.

As you are not from or in the USA you would instead need to fill out a W-8BEN form. I would advise contacting the networks to see if they can issue them.

Did u use ur tax number from ur country or did u have to get n ITIN?
Do I need this ITIN? If so, How do I apply?
I have always used my UK social security number, and it has been accepted.
