You can find your FTP login information by going to Websites > Site Manager and then clicking on the Details button for your site. Make sure to set your FTP program to active mode.

For the premium membership (which is the highest level, there is no up-sale from that point!), you are basically paying to support that content, which is great. There are tons of other perks to going premium. You honestly get so much more than what you pay for with premium. It's been one of the best investments I've made so far and I don't regret it at all (though, it did make the wife angry when she found out at first, but now she understands why!)

I'm not sure, but I can assure that the first month of premium membership is 19$, then recurring $49/m after that.

There are (at the time of writing this) two other replies to your question from Sisterdee and CravenATAT - both of which state that the first month of premium is under $20. Also, as Sisterdee mentioned, when you join as a premium member, you will also get the option of changing to a yearly payment, which provides a 39% discount. The yearly membership is pro-rated based on where you are currently in your monthly payment.
Starter package is free. Premium is not- it starts at $19 a month then expands to a higher amount unless you get the year package.
The premium package includes an immense amount of training while the start package has just lesson one
