You can find your FTP login information by going to Websites > Site Manager and then clicking on the Details button for your site. Make sure to set your FTP program to active mode.

There are (at the time of writing this) two other replies to your question from Sisterdee and CravenATAT - both of which state that the first month of premium is under $20. Also, as Sisterdee mentioned, when you join as a premium member, you will also get the option of changing to a yearly payment, which provides a 39% discount. The yearly membership is pro-rated based on where you are currently in your monthly payment.
Starter package is free. Premium is not- it starts at $19 a month then expands to a higher amount unless you get the year package.
