You have to sign up to each program individually, and in some cases, like Amazon, country-by-country as well. It's worth bearing in mind that there's little point in signing up until you have some traffic, Amazon is one of the few that will approve a "barely there" site but they will expect sales within a given time frame and de-affiliate you if that doesn't happen, meaning you'd need to reapply.
