There are MANY membership plugins to choose from. I would check reviews via Google to find the best one. But I would go for a professional paid plugin since you will be handling personal information. You need to be absolutely sure of the integrity of the plugin to keep the data safe and be able to get support when needed.
For the same reason you should go for a professional theme. Check out the Avada theme. I have built a few websites in Avada and the possibilities are huge. They have a pre-made church theme that you can see here:

You do not need it. Use keyword in the file name + some extra description. Since keyword must be unique for each post, the image file name will be unique as well. This is what I do, and this is a good SEO practice.
