(1) Have you ever ran into a deadlock searching for a plugin? Sometimes it is just not available in the WordPress Plugin Directory. What you can do is to make a search for it in Google and download it unto your pc. Then upload it unto your WordPress website.
(2) An aspect of writing content with intent, is to turn an informative article or video into a freebie and offer it to visitors. Find out more from the image below.

** PS. An image with CTA for a visitor's name and email in two places in a post is acceptable.
One pop-up should not put off a visitor.

I don't understand what it is you are showing us with this image. What are you offering?
This looks like a blog layout, based on the text input, yet if the image is used as a CTA, there is no link.

The Blog post image should have been in its own box. It is not be linked the way it is in the image. (Im trying to use new tools for doing this I could not get it very well, but in the actual result, it should be beside the first paragraph in its own frame).
For the CTA, There will be a link through an autoresponder like Aweber, which is not mentioned in there. So you'll have an input box with space for email address and name which you submit.
The only thing I realise is most of the time it will be in the form of an image which appears once or twice in the post. Pop-ups appear once.
Re (1) Really...did not know that although I don't think I am that sophisticated yet to desire a plugin outside what is available. Good advice however.

Hugh, you can use a sidebar widget, or add a linked image to your post content, if that makes it sound any easier...grin.
