Are you filling in the right form? For people outside the US, it should be the "W-8BEN".
That is the form I tried initially - but it rejected it.
There are one or two things I'm unsure of though...
- I've just put "other" in the "business type"
- would I be "exempt from back-up withholding"?
- is my tax number my NI number? Or should I leave it blank?

For business, I selected "Individual".
Generally, "back up withholding" only applies to US citizens, or people living in the US. Thus in your case, you should be exempt.
Your tax number has to be included to verify that you are not from the US (as far as I can remember). Your Tax number should be the "reference number" that you use to pay your taxes in the UK.
And that "reference number" can have other names, depending on where you live. So you might want to check with the taxation authority in your country.

Well, you're not supposed to use the affiliate programs for your own use unless it is explicitly stated that you can in the agreement, I'm fairly certain that with Amazon you cannot. I'm not suggesting that this is what you did but even your mums name or location may trigger something in Amazon.
Did your mum order from your IP address or have anything shipped to your address, this could cause a breech?
There is also this, are you certain she purchased items that you "should" be paid commission for?
I would contact Amazon and say - what the ... is going on, but that's me.

There is a minimum payment amount before they send it to you. I'm not sure with Amazon but most are 50 bucks. Check your adfilaite agreement with them.

If you login to your AdSense account > Allow & block ads > manage (top, right of left sidebar), you'll be shown a page where all your 'owned' web sites are listed. Make sure all the sites you publish AdSense are listed there - but you don't need to list individual pages.
Next, click on "Access and authorization" (left sidebar), and tick the box adjacent to "Only allow certain sites to show ads for my account". Verify that list is accurate as well.

There have been issues with some plugins altering affiliate links to divert income. The one that's currently creating a stir is Shareholic - see the blog by Jovo (jvranjes) at Warning about Shareaholic plugin for more.
Bottom line: if your affiliate id isn't in the Amazon link, you won't receive credit for the referral to Amazon. The only exception I'm aware of is if your Amazon affiliate link has been shortened by goo.gl, amzn.to, bit.ly, or some similar service.
Bummer.

I would do what Tony suggests. Contact the support team for assurance. Just to clear out any uncertainty.
