With Disclosure Policy (dot) org creating your own disclosure policy is simple and can be done in 6 steps. All you have to do is follow the survey-like questionnaire and fill out the appropriate fields.

Once you've completed the 6 steps the next thing you need to do is to copy the disclosure that they give you at the end.
