I find GeneratePress theme to be very fast. I try to keep my plugins at an absolute minimum. The only point I have not addressed in your post is database maintenance and cleaning.
Are most people doing that regularly and what are you using. WP optimize is getting very mixed reviews.

One thing I do which helps my speed and end-user satisfaction is use an adaptive image service. I use the ShortPixel Adaptive Images WP Plugin. It changes the size and resolution of your images so they can be optimally displayed on any device (iPhone, tablet, destop monitor, etc.) I have tested it on all the commonly used devices at home and at work.
Everything always loads quickly (they have their own CDN) and looks great. is very reasonable priced. I use the Kraken Image optimize, which is set to automatically optimize image size ("Krackem All") before they are sent to ShortPixel. I do this to make the whole process even faster and because ShortPixel won't charge for images optimized by less than 5%, so you can get a lot of freebies. They will also lazy-load images for you so you can get rid of your lazy-load plugin.
I bought 10,000 image optimizations for 10 dollars and it will probably take me over a year to use that up. They also give you 100 free images a month. Thier product support is outstanding. If anyone wants the URL for a ShortPixel coupon then just send me a PM. I sound like a ShortPixel salesman so maybe I should join their affiliate program :-)

The other thing I do is use a lazy-load for video plugin, which can speed up your site tremendously if you embed a lot of videos. The lazy-load creates a thumbnail from the first frame of the video that users can click on, which then quickly loads and plays the actual video. I just posted an article to my site that had 2 videos and it had an average Google PageSpeed of 95 -97.
