How do you insert missing alt attribute into an image that is missing it?
There are two ways to do this:
1. In the Media Library: (#1 below)
Open the Media Library and hover over the image's title. Click on "Edit" and add the text where indicated. This will become the default text that will be used any time the image is placed on a page or post. (Any current uses of the image should not be affected.)
2. In the place(s) where the image is already in use:
Open the page or post that contains the image for editing and click on the image to select it. Click on the "Edit" icon. (#2 below).
On the new page that opens, insert the alternate text where indicated (#3 below). This text will be used only for this instance of the image.
