Do I need to press get QSR button for every keyword? It does not automatically show the QSR unless I specifically click on it
Need is relative - if you think having to press a button is worth another $19/mo, then yes. In my book, no.

Yep, I haven't bothered to pay for it yet. I haven't found the need to.
I am not sure on pricing though as I have not looked into it.

How come I do not see the keyword rich content page on the Site Content page
It only has a word count because the keyword depends on you. So there's no way to really do a template other than having a word count and headings goal.
