Hi - yes, you need an Affiliate Disclosure by law. There is a template in Site Content you can adapt.

Hi - the template in Site Content has the correct wording for Amazon. The disclosure needs to be clearly visible before your potential customers see any of your affiliate links. Therefore, it is better to add it to your top menu navigation bar.

These are questions best asked of your Amazon Associate support staff. So once you join, go to their Contact page and look for the Associate support information. They offer a telephone and/or email means of contacting them. I've always found it easiest to contact them by telephone.
