From my understanding. If you're using adsense and following the guidelines it shouldn't hurt your SEO.

Try not to obsess over keywords. Find one you like, find several related ones.
For the one you want to rank, use it in the title and somewhere in the first 100 words.
Try to fit the others into your content but if they don't work don't worry, just write good information that your audience will like. Focusing on what the search engines want may help but ultimately, you are writing for people not the search engines.
Search engines do not buy.

You pose a good question. Here is my take. A keyword is generally the phrase that you want to rank for and write an article about. You will get ranked for the keyword using your example: "would you like an apple". But, if you mention in your article (content) something about bananas, then you could get ranked for "would you like an apple or banana" as this phrase is a related keyword or LSI as Jay would call them.
Google would rank you for both keywords because they are related. You always want to write about your keyword and be sure to have the keyword in the title of the page or blog and in the first paragraph of your content. As you write naturally, you could also get ranked for LSI keywords to your article.
I would just focus on the main keyword and write about it. If Google should rank you for LSI's, then you have a bonus as you will get more traffic going to your site from both.

I was wondering about your final comment...LOL! See, Jaaxy is based on results from people using exact words/phrases. The first one has a high QSR because it makes sense (grammatically). The second doesn't even qualify because it is an incomplete sentence. Finally, your third phrase is a totally different idea/thought. So it seems more people are searching for "would you like an apple" and naturally, the QSR is the highest. Only the first is an option. Make sense?

What would happen if I just made a page full of random keywords and just linked these keywords to other pages on my website. The article of course would not make any sense but j
All the already mentioned will happen if you tried to put even 10 keywords...let alone 1000 keywords
Google will recognize that you are doing this and rank your page very low or blacklist it. Same goes if you try to hide the keywords by having the font color match the background. Good question, though.
