As per Diane, there is one in site content, that you can adapt, be aware that when you go to post it though you will get a warning that it's duplicate content... don't worry about this! LOL
I should explain that I use my own software and web host to host my website so I won't get any warnings. Thanks though.

Since you will be collecting email addresses, make sure that you have GDPR in place.
