I want to capture leads for follow up after OPT-IN, to submit follow ups to my Leads Prior to input to WA. When leads are sent directly to WA, means that I have no information
You will need to sign up to Aweber or another email provider, and ad an optin box to your website.
You can offer what's called a "lead magnet" such as an ebook, free report, checklist or video training in exchange for their email.

I have several website so I use a couple of things. Autoresponders like Getresponse or Aweber. Also mlsp. Free training, ebook, or any offer will attract leads.

I do not have capture page for upgrade from free trial to Premium Member. Have 25 trials but only 1 conversion to premium. Realize that I don't have a capture page for upgrade
If I understand you correctly, you mean that you want to have a page where people can immediately signup to premium?
If that is correct, you can try going to "WA Affiliate Program" located on the left panel, then "Links & Tracking", then select the link "Premium promo"

Are you referring to the automated message that would get sent to your referral upon upgrading to premium?
Here's a basic email that you could modify to your liking in your messages area:
Title: Congratulations on Upgrading to Premium!
Subject: Hi {firstname}!
Just wanted to drop by and say congratulations on going premium! You have made an awesome decision. It has changed my life in many ways and I am sure it will bring amazing things to yours as well.
