Because search engine cannot evaluate images or other graphical elements, there are some important points that you need to consider when you use them. First, however, it should be briefly noted that the data size of images and multimedia elements should be kept as small as possible. Although now a large proportion of your potential visitors use high-speed connections, there are still many users who do not have these connections. An image should not be larger than 100-150 KB for this reason.

Give each picture a short text alternative (alt attribute), a more detailed description (title attribute). The alt attribute actually has the function to provide an alternative text to the visitor when the respective images cannot be displayed for some reason. This is, for example, the case when browsers not support certain files or the automatic loading of these files has not been activated. The alt text is also displayed when the visitor with its mouse moves over the image. It should therefore contain a brief summary or description of the image displayed. As of right now, there’s no direct relation between image captions and search engine rankings (that I know of).

Does it help if the file name also contains keywords?

You should put a descriptive filename, if it's also a keyword, even better.
