Should I delete the wrong account url on google adsense? Will this affect the correct account name? I opened a new account with the correct url .

If I am not needing anymore, I would delete. Then I can input the correct one. Worth emptying cache.

Why do keep getting this error.? I have checked my titles.
Focus keyphrase doesn't appear at the beginning of SEo Title.
I ignore that section as there are huge bugs with that.
I have had posts that is 100/100 and then when I add my keyphrase the score drops down and it states the same as yours.
It doesn't appear in the title or first paragraph etc.
When I delete the keyphrase it goes back up to the original score.
The main part is to write unique content for your readers not for the search engines of which AIO is mostly based on, add your internal and at least one external link and write naturally the scores are only a guideline

Up to you but I don't bother with the keyphase the main one you should just focus on is the one in the keyword title and within the article as per the trainings given here.

To update your domain DNS settings to point to WA. Here are the DNS settings.
