I'm thinking about trying to sell coffee by independent roasters, but they don't have affiliate programmes of their own. Does the link have to always be set up by the merchant'

In order for you to make any money promoting a companies product you have to have a business relationship with them. Which means they want you to promote their product and you want to be paid for it. Which you may want to really have this down in writing because you may get cut out if it's not, so be cautious as you develop this.
If there is not a link on their web site that links you to their affiliate program, they don't have a program available to the general public. BUT that doesn't mean they don't have an affiliate program because you can call them, email them through the "contact us" form and request to become an affiliate of theirs.
Some companies reserve this feature for those that ask. Most of the time they also want you to be a seasoned Internet marketer so you may have to show them that you can really bring them a lot more business by allowing you to be their affiliate.
Dot your I's cross your T's move with caution.

No, you can't do that one-sided, you can call them and ask if they want to start an affiliation with you, and if they can provide you a code to add to your link, I did that with a Dutch company, who hasn't worked before with affiliates, and they will provide me a special code next week.
