With my intention to bring safety tips and awareness, below you will find the twelve most common Human Factors Causes of Errors, Injuries and Fatalities:
In Aviation, like in many other industries, we believe and practice "SAFETY FIRST".
Here are the twelve main causes, which are also known as the "Dirty Dozen"

Thank you Nattawut.
The problem is that even though we are rational creatures and learn from early age the do and the do not, we still have the tendency to create our own world and start loosing contact with reality. I read long time ago that we are the only animals that make the same mistake hundreds of times.
