When you repurpose an old post and redsate it to present, what do you do with comments from the originally post content?

visitors are smart, they will notice the "updated" time line- so you leave it as it is. Plus unless you have a new stream of commenters coming in it is better to leave comments on a blog post for SERP purposes

If you edit the post (not republish it), add on top of it ,that it is an update with the present date (for example "updated on 18th of April 2021"), so people understand why the comments are older.

Yes but they are dated back when the post was written, so they show a date prior to the date of the currently repurposed post.
