Hey guys just signed to a new affiliate program and filling all my details out. It's asking for a supplier form and i am unsure where i get one

I've never run across the need for a supplier form for any of the affiliate programs I've signed up for.

Hmm, it sounds like you may be looking at what is required to be a vendor instead of an affiliate/associate?

What is the best way to approach a affiliate program?
Couple of questions before I can answer this:
1. In-house program or network?
2. Just asking to join, or asking for something special?
I work with about 7,000 different merchants, so this is totally in my wheelhouse.
Mainly Network from my website, and just simply asking to join so i can promote there products
First before going to an affiliate program. Make sure your affiliate disclosures, disclaimer, and privacy page is on your site. Plus as RoseAnn said you need content on your site, Not necessarily a whole lot but you do need some. Some programs also want you to use your domain email so make you have one of those.
Apply to programs that fit your audience. Be sure to read any rules or requirements before and after applying. Some programs you need traffic, like 25k sessions, while others you don’t need anything.
There are also some affiliate programs that want to know your plans of promotion plus your traffic. Just be honest and most of the time if you meet the program requirements they will approve.
Other than that, you just go and apply. Wait for their approval and you’re done. In the case a program denies approval you can move on to another one or try and email them for further details.
Not sure what you are asking. Do you mean how should you design the content, or actually approach potential affiliate programs? The first is all explained in the training. The second is best done once your website is operational and you have some content, and is also in the training.
There are many affiliate programs, some will want you to apply then they will consider and approve or not. Others will automatically approve.
